To request basic disclosure, each individual is required to complete a request form and return it by mail, attaching copies of identification;

1 photo ID, e.g. passport/driver's license

1 x proof of address e.g. utility bill/bank statement
An DBS application form will be mailed to you for you to complete and return. Once Personnel Checks has received the completed application form, it will then be processed, which normally takes approximately 2-3 weeks.
It is common practice for the Basic Disclosure Scotland Check certificate to be issued to the applicant's home address. However, we can arrange for certificates to be issued to a separate employer or address if required.
