Causes of Femoral Acetabular Impingement

FAI occurs when there is abnormal contact between the femoral head (the ball-shaped top of the thigh bone) and the acetabulum (the socket in the pelvis). This abnormal contact can be caused by structural abnormalities in the hip joint, such as:

Cam Impingement: This occurs when there is an excess bone on the femoral head, leading to a non-round shape. As a result, the femoral head rubs against the acetabulum, causing damage to the cartilage and labrum (a ring of soft tissue that surrounds the socket).

Pincer Impingement: In this case, the acetabulum covers too much of the femoral head, leading to an over-coverage of the socket. This can cause impingement between the two structures.

Symptoms of Femoral Acetabular Impingement

The symptoms of FAI can vary from person to person, and they may develop gradually over time. Common symptoms include:

Hip pain: Typically, the pain is felt in the groin area, but it can also radiate to the buttocks or thigh.

Limited range of motion: Activities like flexing the hip, squatting, or sitting for prolonged periods can trigger pain and discomfort.

Clicking or locking sensation: Some individuals may experience a clicking or locking sensation in the hip joint during movement.

Diagnosis of Femoral Acetabular Impingement

To diagnose FAI, a healthcare professional will typically perform a thorough physical examination and review the patient's medical history. Imaging tests, such as X-rays, magnetic resonance imaging (MRI), or computed tomography (CT) scans, may be ordered to assess the extent of the hip joint damage and identify any structural abnormalities.

Treatment of Femoral Acetabular Impingement

The treatment for FAI aims to reduce pain, improve hip function, and prevent further damage. The approach may vary depending on the severity of the condition and the individual's specific needs. Treatment options include:

Non-surgical management: Initially, conservative measures are often recommended. These may include rest, physical therapy, anti-inflammatory medications, and activity modifications. Physical therapy can help strengthen the muscles around the hip joint, improve flexibility, and correct any biomechanical issues that may contribute to FAI.

Surgical intervention: If conservative treatments fail to provide relief, or if there is significant damage to the hip joint, surgical intervention may be necessary. The two main surgical procedures for FAI are:

Arthroscopic surgery: This minimally invasive procedure involves using small incisions and a camera to remove excess bone, repair damaged tissue, and reshape the hip joint.

Open surgery: In more severe cases, open surgery may be required to access and correct the structural abnormalities in the hip joint.

Following surgery, a rehabilitation program is essential to aid in the recovery process. Physical therapy plays a crucial role in regaining strength, flexibility, and function in the hip joint.
