To create an end portal, place three frames from the end Portal on each side of the portal. Proceed to then turn in a circular direction and then place End Portal frames on each side. Next you need to click on each End Portal frame with the Eye of Enders in hand and then add the Eye of Enders. Be careful not to fall after you have added the last one.

Step 1: You'll first need to build the frame of the End Portal. To do this, first place three frames from the end portal to form one side of the portal. Continue to turn in a circular direction, and place End Portal frames on every side.

TIP: Make sure the tabs in green at the top of the portal frames face the inside of your portal.

Step 2: Now, click on each End Portal frame with the Eye of Enders and add the Eye of Enders to it.

Make sure you pause before placing the 12th and final Eye of Ender into the last portal empty. This is because, if you put the final Eye of Ender inside the portal, you will automatically be transported to the End dimension.
