Documents required

Making a national visa application an applicant must thoroughly fill out, sign, and send a visa application. For Nlv application documents a copy of the EX-01 form must be completed by each candidate, including all required fields, and signed. A recent color passport-size photo that looks forward and isn't covered up by anything that would mask the oval shape of the face, such as dark or reflecting lenses. A passport that is up to date and in valid status. Both the original and a photocopy of the passport page or pages containing the biometric information must be submitted. The passport must have two blank pages and be at least one year old. Passports dated more than ten years ago will not be accepted. 

 In order to obtain a residency permit, the applicant must present original documentation and a copy of the supporting documentation attesting to their ability to support themselves and, if applicable, any accompanying family members during the first year of residency in Spain, or attesting to a reliable source of income. Foreign documents must be authenticated, certified, or apostilled. If required, they must also have an official translation into Spanish. The accreditation certificate from a private or public health insurance firm licensed to do business in Spain. The insurance policy must cover every risk that Spain's public health system provides protection for.  Applicants of legal age must present both the original and a copy of any criminal record check certificates issued by the country or countries in which they have resided for the previous five years. Foreign documents must be authenticated, certified, or apostilled. If required, they must also have an official translation into Spanish.

Original and duplicate medical certificate certifying that the applicant is free from any ailment that could seriously jeopardize public health as defined by the International Health Regulations.
