1. Brokerage Case Management Model  

Case managers assist clients in identifying their own requirements and facilitating the provision of these services. It is based on the idea that the customer will use the required services once they have learned how to access them and determined what works best.  

2. Clinical Case Management Model

This case management model creates a plan for a variety of services, including physical and mental health care. The case manager works closely with the patients to ensure they receive all available benefits.  

3. Intensive Case Management  

This concept is meant to provide numerous necessary services fast, with the case manager accompanying patients throughout the process. This provides patients with the most individualized attention.  

4. Strength-Based Case Model  

This case management methodology develops a care plan based on the client's strengths, needs, and goals.
