  1. Modifier QK
    Modifier QK is an HCPCS modifier and is officially described as “Medical direction of two, three, or four concurrent anesthesia procedures involving good individualities. “


Description
The thing of modifier QK is to bill for an anesthesiologist who provides two to four lapping procedures.
When a croaker performs medical direction of two to four concurrent procedures with anesthesia services by good individualities, the croaker appends modifier QK.
Exemplifications of good individualities include certified registered nanny anesthetists (CRNAs), anesthesiologist sidekicks (AAs), interns, and residents.
Concurrent means that there's some degree of imbrication in the procedures. Depending on payer rules, one nanosecond of imbrication may be enough for procedures to be considered concurrent.


Billing Guidelines
Medical direction can vary per payer. Make sure to check payer rules regarding what qualifies as medical direction.
Modifier QK limits payment to 50 of the quantum that would have been allowed if tête-à-tête performed by an anesthesiologist or non-supervised CRNA.


  1. Modifier QS
    Modifier QS is an HCPCS modifier and is officially described as a “Covered anesthesia care service.”


Description
The thing of modifier QS is to be added to an anesthesia law when services handed by an anesthesia specialist are defined as covered anesthesia care. This is a combination of an original anesthetic with anesthesia.
Modifier QS indicates that the anesthesia provider, who may be an anesthesiologist or other good existent similar to a Certified Registered Nurse Anesthetist, CRNA, or Anesthesiologist Assistant, AA, performs covered anesthesia care.
MAC involves covering the case’s vital signs while in the operating room, examination previous to anesthesia, the tradition for anesthesia care, and postoperative care.


Billing Guidelines
Modifier QS doesn't impact payment. This modifier is instructional. Use it in addition to P modifiers for anesthesia codes.
Payment for anesthesia claims for modifier QS is 100 permissible.
Report modifier G8 rather than modifier QS if the anesthesia provider provides covered anesthesia care for deep complex, complicated, or markedly invasive surgical procedures.
Report modifier G9 if the anesthesia provider provides covered anesthesia care for a case who has a history of a severe cardiopulmonary condition.
Some payers may pay further for modifier QS as long as the reasons for furnishing MAC are medically necessary and proved in the record.

    Modifier QX is an HCPCS modifier and is officially described as “Crna service with medical direction by a croaker.”


Description
The thing of modifier QX medical billing services for small practices performed by a certified registered nanny anesthetist( CRNA) under the medical direction of a croaker.
When a pukka registered nanny anesthetist (CRNA) performs an anesthesia service under the medical direction of a croaker, the CRNA appends the QX modifier on the claim.


Billing Guidelines
The anesthesiologist furnishing medical direction should use a different modifier, similar to the modifier QY.
Modifier QX limits payment to 50 of the quantum that would have been allowed and performed by an anesthesiologist or nonsupervised CRNA.
The CRNA should use modifier QZ rather than modifier QX if a CRNA performs anesthesia services without medical direction.
Check payer rules regarding what qualifies as medical direction for this modifier.

    Modifier QY is an HCPCS modifier and is officially described as the “Medical direction of one certified registered nanny anesthetist (CRNA) by an anesthesiologist.”


Description
The thing of modifier QY is to bill for an anesthesiologist who medically directed a pukka registered nanny anesthetist (CRNA).
When an anesthesiologist provides the medical direction of one certified registered nanny anesthetist( CRNA) performing an anesthesia service, the anesthesiologist appends modifier QY on the claim.


Billing Guidelines
Report modifier QX for the CRNA who performs the procedure.
Modifier QY limits payment to the anesthesiologist and CRNA to 50 of the quantum that would have been allowed if tête-à-tête performed by the anesthesiologist.
Bill modifier QK for medical direction of two to four concurrent anesthesia procedures.


  1. Modifier QZ
    Modifier QZ is an HCPCS modifier and is officially described as “Crna service without medical direction by a croaker.”


Description
The thing of modifier QZ is to bill a service handed by a pukka registered nanny anesthetist (CRNA) that isn't medically directed by a croaker.
When a pukka registered nanny anesthetist (CRNA) performs an anesthesia service without the medical direction of a croaker, the CRNA appends modifier QZ on the claim.


Billing Guidelines
Modifier QZ has no effect on payment and the allowed quantum is what would have been allowed if tête-à-tête performed by an anesthesiologist. Payment for anesthesia claims for modifier QZ is 100 permissible.
Still, use the modifier QX rather than the QZ modifier, if a CRNA performs anesthesia services with medical direction.
