- Release number, Date of Release, Document name, and Product name. All these are present at the header.

- Summary of the Problem includes a brief description of the flaw or improvement. Then steps to Reproduce When the bug was detected, several steps were taken.
- The Resolution is a brief description of the changes to address the bug.
- Impact on the end-user analysis users' activities or functionality affected by the changes effected.
Impacts of Support will contain:
- A list of particular adjustments that must be made to administrate the software.
- Notes about the installation of software or hardware, upgrades, product documentation, and so on.
